Atacand® (candesartan cilexetil) is an angiotensin receptor blocker (ARB), indicated for the treatment of patients with mild to moderate hypertension
- Worldwide, 26% of the adult population had hypertension in 2004 [1] and 29% of the worldwide adult population is projected to have hypertension in 2025. [1]
- At least 50% of untreated hypertensive patients will go on to develop heart failure. [2]
- The World Health Organisation estimates that approximately 62% of strokes and 49% of heart attacks are caused by high blood pressure. [3]
- The risk of cardiovascular disease, beginning at 155/75 mm Hg, doubles with each increment of 20/10 mm Hg. [4]
Atacand® (candesartan cilexetil) acts on the renin-angiotensin system (RAS), which plays an important role in regulating blood pressure. Angiotensin II, the main effector hormone in the RAS, mediates a wide range of responses such as vasoconstriction, sodium and fluid retention, cell growth, and sympathetic activation. Atacand® binds to the AT1-receptor thereby blocking it's interaction with angiotensin II.This blockade leads to vasodilatation and a decrease in blood pressure. Atacand® is at least as effective as other antihypertensive drugs but with the advantage of the better tolerability profile assocaited with ARBs. The ARBs is the newest class of antihypertensives.
Ongoing research includes the DIRECT (DIabetic REtinopathy Candesartan Trials) Programme which investigates the effects of Atacand® on retinal damage and visual loss related to diabetes. DIRECT is scheduled to report mid 2007.

Dosage in hypertension:
The recommended initial dose and usual maintenance dose is 8 mg once daily. The dose may be increased to 16 mg once daily. If blood pressure is not sufficiently controlled after 4 weeks of treatment with 16 mg once daily, the dose may be further increased to a maximum of 32 mg once daily. If blood pressure control is not achieved with this dose, alternative strategies should be considered. Therapy should be adjusted according to blood pressure response. Most of the antihypertensive effect is attained within 4 weeks of initiation of treatment.

Atacand Plus®(candesartan cilexetil/hydrochlorothiazide) is a fixed combination of candesartan cilexetil and the diuretic hydrochlorothiazide (HCTZ), indicated for the treatment of hypertension in patients who need more than monotherapy.
Atacand Plus® is available as a once daily tablet (16 mg candesartan cilexetil and 12.5 mg hydrochlorothiazide). The dose of Atacand® should be titrated before switching to Atacand® Plus. When clinically appropriate a direct change from monotherapy to Atacand Plus® may be considered. Most of the antihypertensive effect is usually attained within 4 weeks of initiation of treatment.
