  This will depend on where your shares are held:
  If you are a UK shareholder either complete one of the online forms or alternatively call Equiniti (formerly Lloyds TSB Registrars) on 0800 389 1580.
  If you are a Swedish shareholder please contact your broker or if you hold a VPC account call the VPC on 08 402 9000.
  If you are a US shareholder please go to the JPMorgan Chase Bank website or alternatively call them on 888 697 8018.
